Pre-Requirements

Baptism

1. Information Data Sheet (IDS) from the GKK they belong
2. Parish Permit for non-Parishioners
3. Birth Certificate (original and photocopy)
4. Marriage Certificate (original and photocopy)
5. Ninong and Ninang should not be below eighteen (18) years old and should be a Roman Catholic
6. Interview of parents will be done on Thursdays at 8:00 AM
7. Attend Pre-Baptism Seminar, held every Saturday at 5:30 PM

Confirmation

1. Baptismal Certificate

Matrimony (Wedding)

1. Pre-Nuptial Inquiry (to be filled out by contracting parties)
2. Interview (Thursdays at 8:00 AM, morning only)
3. Baptismal Certificate (new copy with marriage purpose annotation)
4. Confirmation Certificate (new copy with marriage purpose)
5. Information Data Sheet (IDS) for parishioners
6. Parish Permit (for non-parishioners)
7. Marriage License from Local Civil Registrar (LCR) office
8. Affidavit of Cohabitation
   at least twenty five (25) years old and the couple is living together for five (5) years and/or beyond with Certificate of no Marriage (CENOMAR) from National Statistic Office (NSO)
9. Marriage Contract (Original and photocopy) for civilly married couple
10. Pre-Cana Seminar (Every 2nd Saturday and 2nd Sunday of the month)
11. List of Sponsors (Complete names)
12. Marriage Banns (three (3) consecutive Sundays)
